Mechanism & Biological Context
Tesamorelin activates the GHRH receptor; ipamorelin is a selective growth-hormone secretagogue receptor agonist. Theoretical complementarity comes from stimulating the GH axis through different receptors, but pathway overlap may also amplify GH and IGF-1 exposure.
A plausible pathway is not the same as a demonstrated clinical outcome. Mechanistic, animal, and human findings are labeled separately.
Human Evidence
Tesamorelin has controlled human evidence and an FDA-approved indication for reducing excess abdominal fat in adults with HIV and lipodystrophy. Ipamorelin has limited human pharmacology data. Strong controlled human evidence for the exact tesamorelin-plus-ipamorelin combination was not identified.
